Java script forms perimeter and area

I have a question I am learning about java forms and I want to make a program based on this form which adds two numbers. Now I made a java-code which calculates the area and perimeter...but it needed to be within a form. Here is a form which two numbers are added. I was wondering how do make it take the area and perimeter when a user inputs two numbers. I saved it as a pic because the tags were giving me errors
